We had the privilege of gathering together for a beautiful memorial service at Healthwin Health & Rehab, thoughtfully provided by Heart to Heart Hospice.
It was a meaningful time to pause, reflect, and honor the lives of those we have had the privilege of knowing and caring for. Each person leaves behind memories, stories, friendships, and a special place in the hearts of those who knew them. 🕊️
We are incredibly grateful to the team at Heart to Heart Hospice for creating such a compassionate and beautiful service. Your kindness and support mean so much to our residents, families, and staff.
Meet Heathwin's 🩺 Director of Nursing 🩺 Jeanette Hernandez
Jeanette Hernandez is a dedicated Director of Nursing whose healthcare journey is rooted in compassion, perseverance, and a commitment to caring for others. Born and raised in Los Angeles, California, Jeanette began her healthcare career in 2003 and later moved to Indiana, where she continued to grow professionally and make a meaningful impact in the healthcare field.
Starting as a Certified Nursing Assistant (CNA), Jeanette built a strong foundation in patient care while learning the importance of compassion, dignity, and respect. She continued advancing her career by becoming a Qualified Medication Aide (QMA) and earned her Registered Nurse (RN) license in 2023, reflecting her dedication to lifelong learning, personal growth, and excellence in nursing.
As a Director of Nursing, Jeanette brings more than two decades of healthcare experience, combining clinical knowledge, leadership, and a passion for improving the lives of others. She is committed to developing strong teams, mentoring healthcare professionals, promoting quality and safety, and creating an environment where patients and families feel supported and valued.
Outside of her professional life, Jeanette’s greatest treasure is her family. She is a proud mother of six children and grandmother of three. Her loved ones have shaped her values of compassion, strength, resilience, and unconditional support—qualities she carries into her leadership and daily interactions with others.
Jeanette also enjoys embracing life’s adventures and creating new experiences. She loves exploring new places, making memories, and finding joy in the journey. Our residents had a roaring good time when Potawatomi Zoo brought some very special animal friends to visit! 🐾
From getting an up-close look at some fascinating creatures to learning fun facts about the animals, our residents enjoyed an afternoon filled with curiosity, smiles, and plenty of “aww!” moments. 🥰
Experiences like these bring the world right to our residents and create wonderful opportunities for learning, connection, and meaningful memories. 💚
